I acknowledge that I have voluntarily agreed to participate in a Wellness Wednesday exercise (the "Activity") offered by AT&T Services, Inc. ("AT&T"). I understand the nature and scope of activities that will beinvolved in my participation in the Activity, and I am in sound health to participate in the Activity.

I acknowledge that, as part of my participation in the Activity, there may be some risks not known to me or notreasonably foreseeable at this time. I voluntarily and freely accept and assume these and any and all otherrisks, hazards and dangers, known or unknown, that I may encounter or to which I may be exposed with fullknowledge, appreciation and understanding of the dangers and personal risks associated with my participationin the Activity. I acknowledge that I alone am responsible for maintaining accident and health insurance,including medical and dental insurance, to cover any bodily injury to me, any insurance to cover my short-termor long-term disability from any injury, and insurance to cover my death, which may result from myparticipation in the Activity.

I acknowledge that the Activity may be transmitted on a live and/or delayed basis and that it may be recorded and/or photographed. I agree to be recorded and/or photographed and consent to the use by AT&T or any of their representatives of my name, biographical details, image, likeness, actions and/or voice, throughout the world, in perpetuity, without further authorization or any compensation, in any media, whether now known or hereafter devised, including, without limitation, broadcast, cable and satellite television, social media, and the Internet (e.g., YouTube, other digital video platforms, etc.) and mobile platforms (including mobile apps), and for any purpose (including commercial, merchandising, and promotional purposes).

For the purposes of this release, "Released Parties" means AT&T, and any of their respective affiliated and related entities, subsidiary entities, companies, successors and assigns, and the owners and operators of the location where the Activity is taking place, and the present, former and future directors, officers, employees,agents, contractors, partners, shareholders, representatives and members of each of the foregoing. I forever release, waive, discharge and covenant not to sue the Released Parties from any and all actions, claims,demands, losses or damages that I, my assignees, heirs, distributees, guardians, next of kin, spouse and legal representatives now have, or may have in the future, in connection with my participation in the Activity,including for injury, death, or property damage, whether due to the negligence or other acts, and however caused, by any of the Released Parties. I also agree that I, my assignees, heirs, distributees, guardians, next of kin, spouse and legal representatives will not make a claim against, sue, or attach the property of any Released Party in connection with any of the matters covered by the foregoing release.

This Release and Waiver of Liability shall be governed by the laws of the State of Texas, applicable to contracts entered into and fully performed therein.

I HAVE CAREFULLY READ THIS RELEASE AND WAIVER OF LIABILITY AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE AND WAIVER OF LIABILITY RESULTING FROM MY PARTICIPATION IN THE ACTIVITY.

FURTHERMORE, I hereby acknowledge that this release and waiver of liability is irrevocable.
